Your card will be sent via U.S. Mail and should arrive in 2–3 weeks.

I have not received my Instant Savings Card. How do I find out if it was sent to me?
Your card will arrive via U.S. Mail 2–3 weeks after you register for the Beyond the Burn program. If you have not received your card and it has been longer than three weeks, please call 1-800-458-2300 to check the status.

How do I activate and use my card?
To activate your card, simply follow these easy steps:
 Visit www.prevacid.com/activate or call toll-free 1-800-561-8503. You will be asked for your Member Number on the card. Remember, your card is not valid until you call to activate it!
To use your card:
 After activation, take your card and your Prevacid prescription to your local pharmacy to receive your instant savings. Remember, your card is not a coupon, so be sure to bring it with you to the pharmacy to receive savings every time you fill or refill your prescription.

I lost my card and the letter it came on. How do I get a new one?
To replace lost or stolen cards or a misplaced letter call,
1-800-458-2300.

What should I do if the pharmacy can't process my card?
If the pharmacy has difficulty processing your card, have the pharmacy follow the steps on the back of your Instant Savings Card. The pharmacy can also call 1-800-561-8503 for processing questions.

My pharmacist couldn't process my card, and I have already paid for my prescription. Can I still receive the rebate?
If you have already paid for your prescription, please use the mail-in coupon that was attached to the letter your card came on. If you no longer have the letter or need further assistance, call 1-800-561-8503.

I have an Instant Savings Card, but I get my prescriptions via mail order. Can I still get the discount? What forms do I need?
To get the rebate, fill out and follow the instructions on the mail-in rebate coupon that was attached to the letter your card came on. If you need additional information about mail-order prescriptions and rebates, please call 1-800-561-8503.

Enclose the following required materials to receive your rebate:
 
  • Completed mail-in rebate coupon
  • Original pharmacy receipt
  • Unused Prevacid Instant Savings Card or a clear photocopy
Be sure to keep a copy of your receipt and rebate for your records.

Important Safety Information for Prevacid

If you suffer from persistent heartburn two or more days a week, despite treatment and diet changes, it could be acid reflux disease. Prescription Prevacid Capsules and Prevacid SoluTabTM (lansoprazole) Orally Disintegrating Tablets are used for the treatment of acid reflux disease including erosive esophagitis and for maintenance of healed erosive esophagitis. Your results may vary. Most erosions heal in 4–8 weeks. Prevacid may not be right for everyone and has a low occurrence of side effects such as diarrhea, abdominal pain and nausea. Symptom relief does not rule out other serious stomach conditions. Talk to your doctor or healthcare professional to see if Prevacid is right for you. Prevacid products should not be crushed or chewed.
